We all have had astigmatic patients who’ve complained about blurred or fluctuating vision when wearing their contact lenses.

This complaint may stem from a variety of factors including:
  • Interaction of eyelids with the lens upon blinking
  • Rotational stability of the lens
  • Lens shifting during rapid head movements
  • Binocular vision disturbance from induced prismatic effect
All of these factors can impact visual outcome and should be considered when it comes to toric contact lens prescribing.
Understanding these forces involves observation of anatomical variation between patients and clinical examination with focus on lid position, globe interaction, aperture size, and the effect of blinking on contact lenses.

There are several current toric contact lens designs:

  1. Prism Ballast Lenses that have a thin apex and thick base.
  2. Peri-Ballast Lenses that have peripheral ramp angles and a thick inferior ballast.
  3. Dual Thin Zone Lenses that have sharp junctions between thick and thin zones.
  4. BLINK STABILIZED® Design Lenses

Prism and Peri-Ballasted Lenses:

These lenses are gravity dependent and can be challenged by eye and head movements outside central or straight-ahead viewing.
These particular lenses have shown to rotate about 2.5 times more than other designs like BLINK STABILIZED® Design lenses.1

Another major factor to consider is the induction of vertical prism.

Contact lenses that rely on prism or peri-ballast design, can induce or create prism within the optic zone.

A study was conducted measuring the mean vertical prism in the central 6.0mm zone of eight commercially available soft toric lenses.2

This study showed that unlike lenses with BLINK STABILIZED® Design which had virtually no vertical prism, these other lenses with prism or peri-ballast design had mean vertical prism ranging from 0.5∆ to 1.2∆.2
